Current Openings

 Care Partner I 

 The Care Partner I is a person-centered compassionate caregiver who directly supports participants in recreation, table activities, arts and crafts, music, community outings, exercise, and educational enrichment. Works cooperatively with Assistant Program Director & Program Director/LPN to provide interesting and stimulating program activities that are appropriate for most participants. 

 Care Manager 
(LPN/RN)

As the Care Manager, you play a crucial role in care planning and case management for all enrolled participants. This position is fostered from a holistic approach in support of the caregiver’s journey and is instrumental in ensuring the optimal delivery of participant—and family-centered care. This, in turn, significantly contributes to the safety and well-being of our participants

 Care Partner II
(Certified Nursing Assistant, CNA) 

 The Care Partner II is a person-centered compassionate caregiver who directly supports participants in recreation, table activities, arts and crafts, music, community outings, exercise, and educational enrichment. In collaboration with other members of the Care team, the Care Partner II provides care services to participants including feeding, toileting and assisting with their ADLs (Activities of Daily Living). Works cooperatively with Assistant Program Director & Program Director/LPN to provide interesting and stimulating program activities that are appropriate for most participants.
